3-3-3. PETITIONS-CONSENT.

The Petition shall set forth in simple and brief language the facts which bring the child within the jurisdiction of the Court, as provided in this Code.  The Petition shall further state:

1. The name, age, and residence of the child;
2. The names and residence of his parent(s);
3. The name and residence of his guardian, if there is one;
4. The name and address of the nearest known relative, if no parent or guardian is known;
5. The name and residence of the person having physical custody of the child.
